A report on the sinking of the BC Ferry Queen of the North blamed human error for the disaster. An internal company report said the bridge crew failed to make a critical course change before the ferry plowed into an island on March 22nd, 2006. BC Ferries President David Hahn said the result confirmed there was nothing mechanically wrong with the ship. (Ninety-nine people were rescued and two are still officially declared missing.)
