On October 15th, a judicial inquiry concluded decades of incompetence, neglect, greed and dishonesty by a succession of owners, engineers and municipal officials led to the deadly cave-in at the Algo Centre Mall in Elliot Lake, Ontario, in 2012. Two women were killed when part of the rooftop garage collapsed after decades of rust-causing water and salt penetration. Seventy-one recommendations were made to avoid a similar tragedy. One impassioned Elliot Lake resident noted only one person -- an engineer -- had been charged in the disaster. Eighty-year-old Ed Eckert demanded to know how long it would be before other charges might be laid. (Commission officials noted the inquiry operated under different rules than criminal proceedings and said the process was not about charges but about finding out what happened and making recommendations to prevent future tragedies.)
